CONSENT AGREEMENT FOR PARTICIPATING DOCTORS
This Consent Paper (“Agreement”) is made between Doctors At Home (the “Organization”) and the undersigned Doctor/Medical Professional (the “Doctor”).
By signing this Agreement, the Doctor agrees to the following terms and conditions:

1. Purpose of Engagement
- • The Doctor will provide in-home medical consultation, investigation, diagnosis, and prescription services to patients as dispatched by Doctors At Home.
- • The Doctor understands this is not an employment contract but a professional service agreement on a flexible/rotation basis.

2. Visit Guidelines
- • Doctor will visit the patient’s residence as per the assigned schedule/dispatch order.
- • Each visit may include history taking, physical examination, basic diagnostic tests (BP, glucose, etc.), and prescriptions within the Doctor’s professional scope.
- • In cases requiring higher care, the Doctor must refer or escalate the patient to appropriate hospitals/clinics.

3. Responsibilities of the Doctor
- • Carry valid BMDC registration and professional identification during visits.
- • Maintain professional conduct, confidentiality, and ethical standards in all interactions.
- • Use medical kits and tools responsibly and ensure patient safety.
- • Report visit details in the Doctors At Home system/app after each consultation.

4. Responsibilities of Doctors At Home
- • Provide dispatch schedule, hotline support, and emergency escalation protocol.
- • Ensure fair compensation and timely payment for services rendered. An honorarium of BDT 1500 (including transport cost) will be paid per patient visit.
- • Provide medical kits and operational guidance where required.
- • Offer backup doctor support in case of emergencies or sudden unavailability.
- • Feature participating Doctors’ profiles on the official Doctors At Home website.

5. Terms & Conditions
- • The Doctor shall not misuse patient information or prescribe medications outside legal and ethical practice.
- • Doctors At Home reserves the right to suspend or terminate engagement in case of malpractice, misconduct, or breach of agreement.
- • The Doctor must comply with Bangladesh Medical & Dental Council (BMDC) regulations at all times.
- • The Doctor agrees to follow rotational duty plans as set by Doctors At Home management.

6. Risk & Liability
- • Doctors At Home is a facilitating platform and does not assume liability for individual medical decisions.
- • The Doctor carries personal professional responsibility for medical advice and prescriptions provided.
- • Doctors At Home will not be held responsible for unforeseen patient outcomes, complications, or emergencies beyond the agreed scope of service.
- • Safety during travel and home visits remains a shared responsibility between the Doctor and the Organization.

7. Consent & Acknowledgement
By checking the box, the Doctor confirms that:
- • They have read, understood, and agreed to the above terms.
- • They consent to participate in the Doctors At Home program.
- • They accept the risks, duties, and responsibilities associated with home visits.
